Medical Records Release Request

If you are requesting a copy of your child’s medical records from our office please email your request to firstname.lastname@example.org. In your email please include the following: your child’s name and date of birth, any specific part of the record requested and for whom the records are being requested (i.e. insurance, specialist, personal). The copied medical records will be available for pick-up in 3 business days. If you need the records sooner than this please contact the office write my essay now directly. A processing fee for medical records is due at the time of pick-up.
ADHD Assesment (Vanderbilt Assessment Scale)
If your doctor has requested you to complete the Vanderbilt Assessment Scale to evaluate your child for possible ADHD please click here to download the appropriate forms. Please return the completed forms to our office at your earliest convenience. At that time, you will also need to schedule an appointment to discuss the results of the assessment with your doctor.

Transferring Patient Forms

As a reminder, our physicians strongly believe in and encourage the use of vaccines as recommended by the AAP and CDC. If you are choosing not to vaccinate your child, we suggest looking for an office which more closely shares this philosophy.

All co-pays, charges and account balances will be due at the time of each of your child’s appointments. We accept all major credit cards, personal checks and cash. If you have any questions regarding charges, previous bills or insurance questions please contact our office at email@example.com.
Please call to schedule ALL appointments in advance. Walk-in patients are strongly discouraged and will experience significantly longer wait times than those patients who have scheduled their appointments in advance. Walk-in patients will be seen by the next available doctor. If you have not scheduled an appointment in advance we will not be able to accommodate any request for a specific doctor. We appreciate your understanding and patience in this matter.
